I have the first generation, so cannot comment directly. But here are a couple answers I found online:

"I have heard a few different solutions to this. I had the same problem. The solution that worked for me was to dial my own number from the iPhone. It had me select a password for voicemail, set up my greeting, and listen to an "orientation." After I hung up that call, iPhone asked me for my (numeric only) password I had just created with AT&T. Typed it in... all was cool. Good luck. (This one annoyed me too -- to the tune of a couple hours.)"

"Ok, I solved my problem. The symptoms were: Voicemail button unresponsive. Dialing 1 unresponsive. Dialing my own number just said "voicemail not setup. Goodbye" and then disconnected without allowing setup options. I called 611 for ATT help, then followed the cues to reset my voicemail password. It sends you a text message with the new password. Voila, the voicemail button magically works now. Hope this saves someone else the frustration."

"1. Visual voicemail is COMPLTELY reset when you sign up for the iPhone 3G plan. Your outgoing message and saved voicemails are erased
2. After you get your iPhone 3G, it should recognize that your voicemail isn't set up and prompt you to do so. If it doesn't, go to the voicemail tab in the Phone app and you should have a blue button that reads "Set up voicemail". It will be quick and painless."
