If it's a dual sided swing arm I've seen the adjustment bolts strip after rear impact. If that happens the adjustment set-up will need replacing as no amount of torque on the axle bolts will keep it in adjustment.

If you adjust it and it won't hold adjustment these are the dudes that can betray you. They simply slide in the end of the swingarm. Sorry for chitty cell phone pic.

Great, just found out the case saver that I just bought can't be installed. 

The case saver with this chain/sprocket combo rubs

EK MVXZ Series X-Ring Chain

620M 2004 Dark i.e.; ~ 57K miles (all me);  Looking to swap out engine now.

bad, bad....  is your chain clapped out ??  tell me you're not using a clip style master link vs a rivet version. 
Sounds like things are falling off your bike and you really need a new one : )
