My sister has an 01 4Runner and if that thing has survived her for the past 7 years, it's indestructible in my book. 

Sounds like it might be a winner for my wife!   

The other concern is that if she gets knocked up again and has twins- we would be hosed w/o a third row of seats. I guess I could always strap her to the roof rack.

01 4Runner here with 150k+ miles on it.  I just had the tranny rebuilt though which was a hefty pricetag, but that might have been my fault for never changing out the transmission fluid and i have offroaded it pretty hard on the sand dunes around me.  However before that no serious issues just changed the oil every 3k and it was good togo.  I love it to death!  I'm hoping for another 100k out of it.

You could get a current generation 4Runner with the third row option.

Not a zillion available, but they are out there.
