OK, I can understand forgetting to tighten a lug on a wheel, maybe even all the lugs on a wheel, but all the lugs on all the wheels? I smell

I wouldn't say that, lots of people change wheels at the track, they usually swap all 4 over and walk around with a torque wrench then tighten the wheels, could have gotten distracted and forgot the last step.

biggest problem is the car was either poorly inspected or not inspected at tech..

One of my friends had that (well, one came off) happen on his old Corrado. He had put new rims on that required longer lug studs, but they just slapped the wheels on anyway. Going down the highway the front wheel flew off and tore the quarter panel off. It might have been the same thing here - but for ALL FOUR to fly off simultaneously is just the worst type of hilarious bad luck.

I had a friend do that with his 98 Z28. Car was in the garage for the winter. One particularly nice day he takes it up the street for some gas. The lugs were finger tight. He made it a mile up the road before the rf wheel went sailing. It ripped the fender clean off! No  [drink] involved. Dummy.  [laugh]
