I haven't had a bike for the past few years, mostly because we're still trying to recuperate from a few years of relocation. We are finally selling our current home and will be moving full time to Atlanta! Along with the move, I'll be pushing for a promotion at work. If the planets align, my reward will be an 821 Stripe. That being said, I'm already looking at the mod possibilities.
From the 821 owners, what are the must do other than exhaust? I've heard complaints about the peg placement. Also high on the list would be the tail tidy.

If you go for exhaust, go with the Termi and upmap . . after that, do whatever you want as long it is a slip on . . . regarding the tail tidy, I personally like the bike whole, there is a shorter Ducati Corse version in Aluminium . . .and LED blinkers . . . just remember that front turn signals ARE NOT whiting the headlight assembly but, below with brackets to "extend them" outwards . . .
Now, Red w/White Stripe or White w/Red Stripe . . .

Red /w white stripes!!
I'm not going to deal with the exhaust at first. I was mostly worried about having to invest in new rearsets from the get go considering the heal issues I keep hearing about.

Agreed! He showed me that when I bought mine! Needless to say, I didn't buy them. After ~4000 miles, the black coating (paint? PC?) on the rearsets are starting to wear off where my boots rub. I have gotten used to riding mid foot instead of on the balls of my feet.
I bought my 821 a few months ago. I just did the tail tidy (literally, put it on this morning). I used the MotoDynamic fender eliminator and the integrated tail light.
So far, I really like it. Looks very clean, the tail light is quite bright. Everything lined up nicely and everything needed for install was included in the kit. No cutting and splicing of wires was needed. I got mine off of ebay and because of how close the seller was, I received the package less than 24 hours after I ordered it!

This sounds like a dream purchase! I was looking at the MotoDynamic one, featured in a video, and it looked great. As I get closer to pulling the trigger I'll ask you again if you're still happy with the purchase. This will be one of the first purchases, along with bar-end mirrors. I'm probably going to start riding mid foot anyway since now with ABS I can actually start using the rear brake.

If you bought their integrated tail light, does that mean that you did not have to install the turn signal brackets that came in the kit? Or is there a full kit that includes and tidy and the light?

I bought them separately. But the brackets for the stock turn signals do not have to be installed.

That's what I thought.
Another thing that will be bought eventually is a clear wet clutch cover. Thank God my company pays me a portion of my bonus every quarter. I would waste the whole thing in one shot.

So you don't replace the whole ECU, just the map? That's cool. Is that the case with other full systems like the Akrapovic?

at the time of this writing or previous ones, Ducati has NO plans to make a FULL EXHAUST for the M821 . . . really makes a difference in all 3 modes . . . plus, being a Continental/Siemens-VDO unit, Rexxer might already have maps for it already . . .
