Being trapped, somehow or by somebody, and being force to eat your own limbs in order to have the chance to possbly survive ... then dying anyway.

Being run over by something like a train where it cuts you in half, but the train's weight keeps you from dying, but as soon as they move it you die.

Slipping in the shower, getting paralysed, nobody knows what happened to you and realizes that you are missing and your pet cats eat you alive.

Once what ever cut you in half completed the job, I wouldn't think you could feel the other half and would DEFINITELY be in shock

Once you were paralyzed you couldn't feel them nibbling your body anyways.  You could just blow in their face to keep them away from yours.  Everyone knows how much kitties hate to have their face blown in.

it's more interesting if it is YOUR worst way to die.  not just dying in general, but what scares the shit out of you. 

being eaten alive takes the cake for me.  i used to go scuba diving a lot until i saw Jaws.  make the beast with two backs that.

The running joke for me is that I am impervious to death, but not getting make the beast with two backsed up.  I don't actually think that, but its starting to kind of seem that way.
So, I am more afraid of nearly dying and pretty much being a head, like Million Dollar Baby, than of actual death.  Your time comes when it comes.  If it hurts, it hurts.

My worst way huh, ok then.  Malled by an animal while hiking in the middle of nowhere and bleeding out.  Or being quickly split in half horizontally across the torso.

'Sfunny.  With respect to the Jaws creep-out, I'm more comfortable under the surface, able to see around me, and seeing sharks, than I am floating on the surface, unable to see well below me, thinking about sharks.

So:
going on a night dive, flashing around my dive light, and seeing a shark tooling past hunting fish = cool.
floating on the surface at night, wondering if there's maybe a Ginormous Flesheater™ looking up at me = not cool.

yeah, that would freak me too.  and i have a built-in shark repellant -- i shit on site.  usually chases them away pretty quick.
